Why Korea's BMI cutoffs are lower than the WHO's

If your health checkup in Korea shows a BMI of 24, the report says "pre-obese." Type the same number into a calculator from abroad and it says "normal." Is one of them wrong? No. Korea uses the cutoffs of the Korean Society for the Study of Obesity (KSSO), while most international calculators use the World Health Organization (WHO) classification.

BMI is weight relative to height

BMI (body mass index) is your weight in kilograms divided by the square of your height in meters. At 170 cm and 70 kg, that is 70 ÷ (1.7 × 1.7) ≈ 24.2. Anyone can work it out from height and weight alone, and it tracks body fat reasonably well across many studies, so it is the most common first check for obesity.

The two classifications side by side

The WHO defines adult overweight as a BMI of 25 or more and obesity as 30 or more. In its 2022 guidelines, the KSSO defines a BMI of 23 or more as pre-obese (overweight) and 25 or more as obese, and splits obesity into three classes. These cutoffs are unchanged from the previous edition; the 2022 update put more weight on basing them on where obesity-related diseases actually rise.

The same BMI of 24.2 is pre-obese in Korea and normal under the WHO cutoffs. Korea uses six bands; the WHO uses four broad ones.

Asian populations face risk at a lower BMI

There is a reason Korea's cutoffs are lower. In 2004 a WHO expert consultation reviewed BMI cutoffs for Asian populations and published its findings in The Lancet. Many studies showed that the links between BMI, body fat percentage and health risk differ between Asian and European populations, and the consultation concluded that a substantial share of Asian people have a high risk of type 2 diabetes and cardiovascular disease below the existing overweight cutoff of 25.

It did not find a single number that fits all of Asia, though. The BMI at which risk starts to appear ranged from 22 to 25 across populations, and the BMI for high risk ranged from 26 to 31. So the WHO kept 25 and 30 as the international classification and proposed 23, 27.5, 32.5 and 37.5 as additional points for public health action, leaving each country to set its own risk cutoffs from its own data.

The KSSO's use of 23 and 25 follows that approach, based on where obesity-related diseases clearly increase in Korean data. So rather than calling Korea's cutoffs "stricter," it is more accurate to say they are set where risk actually rises for Koreans.

What BMI cannot tell you

BMI is convenient, but because it uses only height and weight, it has clear limits.

  • It cannot tell muscle from fat. Someone with years of strength training can have a BMI over 25 with little fat. Someone with little muscle and more fat can have a normal BMI and a high body fat percentage.
  • It does not show where fat sits. At the same BMI, more fat around the belly means more risk. The KSSO defines abdominal obesity as a waist of 90 cm or more for men and 85 cm or more for women.
  • Its meaning can shift with age. Muscle tends to fall and fat to rise as we get older, so your body composition may change while your BMI stays the same.
  • The adult table is not for children and teens. They are still growing, so they are assessed by percentiles against children of the same sex and age.

How to read your result

First check your band under the Korean cutoffs, and when comparing with numbers from foreign articles or apps, check which classification they use, since the same 24 means different things. If you are near a boundary, measure your waist too and watch which way the number moves over a few months, rather than fixating on a single reading.

If your BMI is high but you train regularly and carry a lot of muscle, check your body fat percentage as well. If your BMI is normal but your waist is over the cutoff, that may be a signal to look at your habits. This note is general information, not medical advice. Talk to a doctor if you need a diagnosis or treatment.
